AANSOEKE

Aandag ouers/voogde:

Alle inligting moet deeglik ingevul word, aanlyn of uitgedruk en ingeskryf.
Nadat dit ingevul is, druk vorm, teken en handig in saam die versoekte vorms soos op aansoek aangedui.
Die vorm moet soos volg oop gemaak word om aanlyn in te vul:
- Moet in ‘n web browser oopgemaak word om te kan invul.
- As die vorm na jou epos gestuur word “save” dit eers na jou rekenaar se dokumente en maak dan oop in jou “web browser”.
Ongelukkig werk die aanlyn vorm nie op fone nie – moet op rekenaar oopgemaak word.
Versekering aansoeke sluit einde Maart.

Attention parents/guardians:

The forms must be filled in thoroughly, either online or printed out and completed.
After completion, print form, sign and submit together with any requested forms as stipulated on the application.
The form must be opened as follows to complete online:
- Must be opened in a web browser to fill out.
- If the form was sent to your email first save it to your computer’s documents and then open in your web browser.
Unfortunately the online form does not work on phones – must be opened on computer.
Insurance applications close end March.
